Fee Schedule/Prices
Change Physical Therapy is out of network with all insurance companies. Payment is collected the day of service. An EOB/Receipt will be given at the patient’s request for submitting to insurance towards patient/family deductible
or for reimbursement depending on your health plan. HSA, HRA or other employer sponsored health accounts are gladly accepted.
